You can print them and fill them out before you get here, or just read through them to familiarize yourself with the process.
KDC is always accepting new students! We are also ODJFS approved!
Child Enrollment Form - Form JFS 01234
Download, print, and fill out this form to enroll your child at KDC. Please note that filling out the form is only one step in the process. You'll need to call and schedule an appointment to visit the KDC of your choice. Our administrators will review everything with you to ensure you're completely comfortable.
Child Medical Statement - Form JFS 01305
Download, print, and fill out the Child Medical Statement to help us take the best care of your child. According to the State of Ohio, we need to know which vaccines your child has received and when, as well as other important information.
Center Parent Information
Download and print the Center Parent Information. Everything you need to know about the Kiddie DayCare should be posted in a conspicuous place, as stated by the State of Ohio and this administrative code.
Application for Child Care Benefits JFS 01138
Download, print, and fill out the Application of Child Care Benefits if you wish to qualify for financial assistance through the State of Ohio. This form must be completed and turned in to the Ohio JDS before applying at our facilities.

Infant Information Sheet
Download, print, and fill out the Infant Information Sheet. This information will help us provide the best care for your infant, including details such as hints to help your child get to sleep and what other 'security' items they like to have. This form is required for all infants according to the State of Ohio.
Family Information For Step Up To Quality Programs

Providing detailed information about your child helps the staff create a positive and nurturing experience while your child is in their care. Please share any details about your child's habits, abilities, or personality that would assist the staff in offering the best support and attention.
School Age Information

Childcare Assistance Application - Form JFS 07200
